Due to an executive order released April 24, 2002 by Gov. McGreevey, certified copies will only be issued if all the following criteria are followed:
1. Complete application with all pertinent information
2. A certified copy of a vital record may only be issue if you are one of the following:
· The subject of the record
· The subject’s parent, legal guardian or legal representative
· The subject’s spouse/civil union partner; child, grandchild or sibling, if of legal age
· A state or federal agency for official purposes
· Pursuant to court order
3. Acceptable identification (photocopy if applying by mail).
a. Current Photo Driver’s License
or
Two (2) forms of I.D. with current address (example: CountyI.D., passport, utility/bank statement, vehicle registration, etc)
And
b. Proof of relationship to the subject on the record- (example: for your parent’s or sibling’s vital record… you must provide a copy of your birth certificate)
3rd Party Representatives (Lawyer, Accountant, Etc) or Non-Eligible Persons:
a. Provide legal documentation (Surrogate’s Certificate, court order, etc.)
or
b. A Notarized Letter from the eligible person allowing the certificates to be released to 3rd party (acceptable identification is still needed from eligible person).
4. Effective April 3, 2008 the fee for a Certified Copy is $10.00 each - check or money order payable to “Township of Brick” or exact amount of cash.
5. Self addressed stamped envelope, if applying by mail.
